MBX Biosciences doses first patient in Phase 3 canvuparatide trial

The NASDAQ-listed biotech has initiated its pivotal oPTimize study of once-weekly canvuparatide in roughly 160 adults with chronic hypoparathyroidism.

MBX Biosciences has dosed the first patient in its pivotal Phase 3 oPTimize trial of canvuparatide, a once-weekly parathyroid hormone (PTH) peptide prodrug, in adults with chronic hypoparathyroidism. The Carmel, Indiana and Burlington, Massachusetts company said the global registrational study will enrol approximately 160 participants, and that the move follows positive Phase 2 Avail data demonstrating durable clinical benefit at one year.

The trial's primary endpoint is a composite responder criterion assessed at week 26. To be classified as a responder, a patient must maintain normal albumin-adjusted serum calcium levels, achieve independence from active vitamin D, reduce calcium supplementation to no more than 600 milligrams per day, and avoid any increase in study drug dose during the final four weeks of the blinded period. Key secondary endpoints include normalisation of urinary calcium excretion and improvements in physical function and hypocalcaemia-related symptoms.

Trial design and mechanism

The study is a randomised, double-blind, placebo-controlled design with a 3:1 active-to-placebo ratio over a 26-week double-blind treatment period, followed by a 78-week open-label extension. The treatment period itself is structured in three stages: a four-week fixed-dose period at 600 micrograms weekly, an 18-week individualised dose-titration phase running up to 1,600 micrograms, and a four-week maintenance window. MBX has said the commercial-intended device will be used throughout, a deliberate design choice intended to streamline the regulatory submission package.

Canvuparatide holds Orphan Drug Designation from both the US Food and Drug Administration and the European Medicines Agency, which affords it certain regulatory incentives including reduced filing fees in the US and a ten-year period of market exclusivity in Europe if approved. The Orphan designations were granted for hypoparathyroidism specifically, an important distinction as they do not extend to the company's broader endocrine pipeline.

Steve Hoerter, chairman and chief executive of MBX Biosciences, said the Phase 2 results reinforced confidence that canvuparatide has "potential to become a new standard of care in the treatment of HP."

Market context and competitive landscape

Hypoparathyroidism is a rare endocrine disorder in which insufficient PTH production leads to hypocalcaemia and disrupted mineral homeostasis. The company estimates there are more than 250,000 patients in the US and EU combined, with around 7,000 new diagnoses per year. Standard of care remains calcium supplementation and active vitamin D, approaches which do not restore physiological PTH activity and carry long-term risks including renal complications and chronic hypercalciuria.

The space has attracted sustained interest from drug developers. Takeda's PTH analogue Natpara (parathyroid hormone 1-84) holds approval in the US for adjunctive treatment of adults with hypoparathyroidism, though supply challenges have constrained access. Ascendis Pharma's TransCon PTH, a prodrug concept with mechanistic similarities to canvuparatide, received FDA approval as Yorvipath in late 2024 as a once-daily injection, and is now commercially active. MBX's once-weekly dosing interval is positioned as a clinical differentiator relative to once-daily competitors, though head-to-head data will not emerge from the current trial design.

Beyond canvuparatide, MBX is developing an obesity pipeline that includes MBX 4291 in Phase 1 and two further candidates in preclinical development, suggesting the company is building breadth in endocrine and metabolic disease. Investors will be focused on interim tolerability signals from oPTimize and any guidance on a potential NDA or MAA filing timeline as the 26-week blinded period progresses.
