Ocugen doses first patient in Phase 3 gene therapy trial for dry AMD
Ocugen has dosed the first patient in ArMaDa3, its global Phase 3 registrational trial of OCU410, a modifier gene therapy for geographic atrophy (GA) secondary to dry age-related macular degeneration. The Pennsylvania-based company, listed on NASDAQ under the ticker OCGN, said the milestone follows receipt of FDA Regenerative Medicine Advanced Therapy (RMAT) designation on 29 July 2026 and a successful End-of-Phase 2 meeting with the FDA's Centre for Biologics Evaluation and Research in the same month.
OCU410 is an AAV5-delivered gene therapy that introduces the human RORA gene via a single subretinal injection. By upregulating RORA, Ocugen says the candidate simultaneously modulates complement overactivation, chronic inflammation, oxidative stress, and lipid dysregulation. This multi-pathway design sets it apart from the two complement inhibitors currently approved in the United States, pegcetacoplan (Syfovre, Apellis) and avacincaptad pegol (Izervay, Astellas/Iveric Bio), both of which target a single complement node and require repeated intravitreal injections throughout a patient's life.
Phase 3 design and Phase 2 data
The ArMaDa3 trial (NCT07770828) is a global, multicentre, randomised controlled study enrolling 237 subjects across sites in the United States, Canada, Europe, and Latin America. Participants are randomised 2:1 to a single 200 µL subretinal injection of OCU410 at a concentration of 5x10¹⁰ vg/mL or an untreated control arm. The primary endpoint is the rate of change in square root-transformed GA lesion area measured by fundus autofluorescence at months 4, 8, and 12. Functional vision is anchored by a secondary endpoint tracking the proportion of subjects losing 15 or more ETDRS letters on low-luminance visual acuity assessment.
The Phase 3 design draws directly on 12-month data from the Phase 2 ArMaDa trial in 51 subjects. At the selected dose, OCU410 produced a 31% reduction in lesion growth rate relative to control (p less than 0.05) in the pivotal population, alongside a 27% reduction in ellipsoid zone area loss measured by SD-OCT. No drug-related serious adverse events or adverse events of special interest were reported. Ocugen notes these figures compare favourably with the 15% and 22% lesion growth reductions reported for the approved complement inhibitors at 12 and 24 months respectively, though cross-trial comparisons carry significant methodological caveats.
Chief medical officer Mohamed Genead said the programme enters Phase 3 with "a dose validated in a randomised, controlled Phase 2 study; an FDA-endorsed primary endpoint measuring the rate of lesion growth; and a secondary endpoint assessing functional vision." The company is targeting a BLA submission in 2028 and is in parallel discussions with the European Medicines Agency on whether the single pivotal trial can also support a marketing authorisation application.
Regulatory and competitive context
RMAT designation, granted to regenerative medicine therapies addressing serious unmet needs, provides access to intensive FDA guidance, eligibility for accelerated approval and priority review, and early discussion of surrogate endpoints. For OCU410, the surrogate anatomic endpoint (lesion growth by fundus autofluorescence) is already endorsed in the Phase 3 design, making the RMAT interaction benefits particularly relevant as Ocugen builds its BLA dossier.
GA affects an estimated two to three million people in the United States and Europe and is a leading cause of irreversible central vision loss in older adults, with prevalence expected to climb as populations age. Outside the United States, no treatment is currently approved. That regulatory gap underpins the commercial case for a one-time therapy and gives Ocugen a potential first-mover position in several major markets if ArMaDa3 meets its endpoints. The broader gene therapy retinal space remains active, with a number of academic spinouts and established gene therapy companies pursuing candidates in inherited retinal diseases, though GA-specific programmes at a comparable clinical stage are limited. Investors will focus on 12-month primary endpoint readout timing and any interim safety signals from the subretinal delivery procedure as the key near-term data points.
