AliveCor KardiaMobile 6L nearly doubles arrhythmia detection in trial

A 350-patient prospective study found the handheld 6-lead ECG device detected arrhythmias in 14.9% of patients versus 7.7% for standard ambulatory

AliveCor has published prospective clinical data showing its KardiaMobile 6L (KM6L) handheld ECG device outperformed standard ambulatory monitoring in detecting clinically significant arrhythmias, with researchers calling for it to be considered a first-line diagnostic tool in patients presenting with unexplained cardiac symptoms.

The M-KEEPS study, conducted by the University of Rochester Clinical Cardiovascular Research Center and published in JACC Advances, enrolled 350 adults experiencing unexplained palpitations, dizziness, or lightheadedness. Participants wore both a physician-ordered ambulatory monitor (either a 24/48-hour Holter or an extended patch device) and the KM6L simultaneously over 30 days.

Clinical findings

Across both monitoring modalities combined, clinically significant arrhythmias were identified in 66 patients, representing 18.9% of the cohort. Traditional ambulatory monitors detected arrhythmias in 27 patients (7.7%), while the KM6L identified arrhythmias in 52 patients (14.9%), a difference that reached strong statistical significance (p less than 0.001). Crucially, 39 arrhythmia episodes, or 11.1% of the total cohort, were captured exclusively by the KM6L and would have been missed entirely by conventional monitoring.

Atrial fibrillation was the condition most frequently missed by standard monitoring: the KM6L detected 23 AF cases that the ambulatory monitor did not capture, while zero AF cases were detected solely by the ambulatory monitor. In an exploratory sub-analysis restricted to patients wearing extended patch monitors for seven days or longer, the KM6L still outperformed those devices, identifying arrhythmias in 23.9% of that subgroup versus 9.8% for the patch alone.

Signal quality held up well in real-world use. Of 6,251 total KM6L recordings collected during the study, 92.3% were rated clinically interpretable, and 88.3% were classified as top-tier readable ECGs, marginally ahead of traditional Holter recordings at 86.7%. Patient usability surveys showed 88% of participants found the smartphone application easy to use, and two-thirds of participants were women, broadening the demographic relevance of the findings.

Wojciech Zareba, MD, PhD, of the University of Rochester's Clinical Cardiovascular Research Center said: "KM6L might be considered as the first line of diagnostic process in individuals with uncertain causes of cardiac symptoms."

Market and competitive context

The result arrives at a moment of accelerating interest in consumer-facing cardiac monitoring. The ambulatory ECG monitoring market is crowded, with established players including iRhythm Technologies, whose Zio patch is widely deployed in the US and UK, alongside Philips and Medtronic in the longer-duration monitoring space. The KM6L's on-demand, symptom-triggered design addresses a well-recognised limitation of continuous monitors: patients whose arrhythmias are transient and infrequent may not experience a symptomatic episode during a fixed monitoring window, leading to inconclusive results and repeat testing.

The M-KEEPS study adds to a growing body of peer-reviewed evidence supporting patient-activated ECG recording, a concept that regulators and payers have been cautiously warming to. In the UK, NICE has issued guidance supporting remote ECG monitoring in atrial fibrillation detection, and NHS integrated care boards have begun piloting consumer devices as adjuncts to routine care pathways. The MHRA-cleared status of KardiaMobile devices in the UK means clinicians in this market face a clearer regulatory path to adopting the technology than in some other jurisdictions.

AliveCor is privately held, so the commercial implications of the study are harder to quantify than for a listed peer. The company says it has accumulated more than 350 million recorded ECGs and has over 250 peer-reviewed articles supporting its technology. For clinicians managing high-volume cardiology outpatient lists, the prospect of reducing repeated short-duration monitoring loops and accelerating symptom-rhythm correlation represents a meaningful workflow gain, provided payer reimbursement frameworks can be aligned to support 30-day patient-initiated recording protocols.
