Myriad Genetics Prolaris meta-analysis backs prostate cancer staging
Myriad Genetics has published results from what it describes as the largest individual-patient-data meta-analysis of its Prolaris Biopsy Test to date, reporting that the genomic assay provides independent prognostic information on top of established clinical scores for men with localised prostate cancer.
The study, published in Clinical Genitourinary Cancer, pooled data from 14 cohort studies covering 8,478 patients, with individual participant data available for 7,924 of them. Researchers found that the Prolaris combined clinical risk (CCR) score was significantly associated with two hard endpoints: distant metastasis and prostate cancer-specific mortality. Crucially, the association held across all National Comprehensive Cancer Network (NCCN) risk groups and regardless of the initial management strategy chosen, including active surveillance and definitive therapy.
The test analyses RNA expression from a prostate biopsy sample, focusing on a panel of cell-cycle genes, and combines that signal with clinical variables to generate a single CCR score intended to support shared decision-making at the point of diagnosis.
What the data adds
Steven Monda, a urologist and clinical instructor at the University of Michigan Health who was a study author, said the findings extended the test's utility into the lower end of the risk spectrum. "CCR is prognostic for prostate cancer-specific mortality, even in low-risk patients," he said. "That's useful in active surveillance conversations, where you want additional information about a patient's individual cancer risk to inform counselling and surveillance intensity."
The claim that Prolaris adds information beyond CAPRA score, Gleason grading, and NCCN classification is the core commercial proposition. This meta-analysis is notable for its scale and for using individual participant data rather than aggregate study-level figures, a methodological approach that generally permits more robust subgroup analyses and reduces heterogeneity bias.
Market context
Molecular prognostic testing in prostate cancer is a competitive space. Myriad's Prolaris competes directly with Genomic Health's Oncotype DX Genomic Prostate Score, marketed by Exact Sciences, and with the Decipher Prostate test developed by Veracyte. Each assay measures a distinct gene expression signature and has its own evidence base, and clinicians in the United States can access all three under current reimbursement frameworks. The ability to demonstrate additive value over conventional staging tools, at scale and across risk groups, is the key differentiator argument each manufacturer makes to payers, network formulary committees, and clinical guideline bodies.
For Myriad, this publication arrives at a strategically important moment. The company has faced sustained pressure on its hereditary cancer testing revenues as competitive pricing has intensified, and it has been building out its urology diagnostics portfolio partly to diversify the revenue base. Prolaris's reimbursement position in the United States is established, but wider guideline incorporation, particularly in active surveillance pathways where clinical equipoise remains, would represent a meaningful commercial opportunity.
The release also references a recently launched artificial intelligence digital pathology feature for Prolaris, though no data or further detail on that capability was provided in the accompanying materials. Guideline bodies including NCCN and the European Association of Urology periodically review evidence for molecular markers in prostate cancer risk stratification; a meta-analysis of this scale may be cited in future updates, though formal guideline endorsement is a separate and longer process.
